MLN Matters reports majority of Home Health improper payments due to insufficient documentation.

While we continue to await further clarifications from the Centers for Medicare and Medicaid Services related to documentation required from face-to-face encounters for durable medical equipment, today they clarified face-to-face documentation needed when the beneficiary requires home bound status and the need for skilled services.
